What is My Aged Care?

My Aged Care is the Australian Government's gateway to aged care services. Through this system, eligible Australians can access funding to receive the help they need at home.

Self Managed Home Care Packages

Many clients choose to self-manage their Home Care Package, which provides more flexibility, control, and choice in how services are delivered.

As a Self-Managed Client, You Can:

  • Allocate your funds
  • Directly coordinate care
  • Save on provider fees

How to Get Started

Register with My Aged Care

Call 1800 200 422 or visit www.myagedcare.gov.au to register and arrange an assessment.

Get Assessed

A member of the Aged Care Assessment Team (ACAT) will assess your needs to determine if you’re eligible for a Home Care Package.

Receive Your Package

Once approved, you’ll be placed in the national queue and assigned a package when one becomes available.

Choose a Provider

You can then choose to self-manage your package and coordinate directly with Leader Care Services for your care needs.
